Passport Services

Business or vacation taking you abroad? The City of Jacksonville is certified through the US Department of State as a Passport Acceptance Facility. We offer the following services:

  • Passport forms
  • Passport photos
  • Expedited and overnight services

Important documents checklist - The City will accept your completed application for a new passport and forward it to the Passport Services section of the U.S. Department of State for an issuance determination. All applicants submitting passport application Form DS 11 must:

  • Appear in person
  • Completed passport application (DS-11), typed or filled out in black ink
  • Proof of US citizenship (birth certificate, expired passport, naturalization certificate)
  • Submit proof of identity (driver's license, military ID, other government issued ID, previously issued passport, etc)
  • Provide a recent 2 inches by 2inches high-quality photograph (Jacksonville offers photo service, see below)
  • Check or money order for City Services made out to the City of Jacksonville
  • Check or money order for USDOS Services made out to the US Department of State. See pricing information below
  • Please note that all documents must be original or certified copy

Adult Passport Book $110 For travel outside the US, any transport
Adult Passport Card $30 The US Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book. The passport card cannot be used for international travel by air.
Minor Passport Book (under 16 years of age) $80 For travel outside the US, any transport
Minor Passport Card (under 16 years of age) $15 The US Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book. The passport card cannot be used for international travel by air.

The non-refundable fee is charged per application for processing by the City of Jacksonville. Acceptable form of payment includes check or money order.
Photos $10.00 Optional - Applicant may bring his or her own photo  (As of July 1, 2016 fee will be $10 for this service)
Overnight Delivery $24.70 Optional - Overnight delivery from the City to the  passport processing center
Photo Copy $0.25 Charge per copy

Passport Book Overnight Delivery $15.89 To request Overnight Delivery, pay the $15.89 fee per application submitted. Overnight delivery service is not available for Passport Cards, which are delivered First Class Mail only.
